Livingston County Health Department Covid-19 Self-Reporting Form
If you have tested positive for COVID-19 or have been exposed to someone who tested positive for COVID-19, use this form to self-report as soon as possible and request a return to work/school letter.
If you are a parent/guardian who tested positive for COVID-19, please use this form to report your positive test results and report any children who are in quarantine due to your positive test results.

Our challenge to you: Personal Growth
We are encouraging all students to focus on personal growth during the closure. What is something that you would like to learn about or learn how to do? Is there something you’d like get better at? A goal you haven’t had time for? Something new you’d like to try? Challenge yourself to commit some time every day to DO SOMETHING and GROW!
Teachers will be participating too! We will be sharing their experiences on our Instagram and Facebook accounts in coming days and we want you to share too! Sharing our experiences with our community is a great opportunity for us to stay in touch as a community through this time. Make sure to post your experiences (photos or videos) on Instagram or Facebook with the #KWoodsChallenge and tag the school (post publicly so we can share it) or send it to Mrs. Pratt (jpratt@kwoods.org). We’ll be selecting some submissions to share on our social media channels!
Enrichment Activities for High School students:
Test Prep – 9th-11th grade:
All 9th-11th grade students can work in Khan Academy on SAT test prep. All students already have an account that they have been using in Advisory. If they need password resets, they should follow the instructions in Khan Academy.

Enrichment Activities for Middle School students:
Prodigy Math: All middle school students have a Prodigy Math account and can continue working on math enrichment through the Prodigy platform. Ms. Hoopingarner will be sending out logins individually to families in case they are needed. Any questions can be directed to Ms. Hoopingarner (choopingarner@kwoods.org).
IXL: IXL is an online learning platform where students can practice math and English skills. Many middle school students already have an account from Mr. Ferguson’s class and they can log in and get started. If your student does not have an account (or needs a password reset), email Mr. Ferguson (kferguson@kwoods.org) to get set up!

Enrichment Activities for all students:
Read, Read, Read! Please don’t underestimate the value in just taking time to read. Reading has so many academic and social/emotional benefits. If you have younger children in your house, read to them, or set up a virtual storytime with friends and family! If you have an active library card, you can most likely check out electronic and audio books – check your library’s website for more information!
Virtually Connect: Visit a virtual museum or watch a virtual performance! Many museums have virtual tours and symphonies and operas are streaming their performances free, along with other institutions doing daily Facebook Live events. Here’s some (but you can do a Google search for lots more!)
Ted-Ed: Ted-Ed has lots of great, engaging, educational videos. Here's a big list.
Khan Academy: Khan Academy offers lots of courses in different subjects with detailed lessons
